The Affordable Care Act, Medicaid Expansion, and the Effect on Advanced Practice Nursing In 2010, the President of the United States signed the Patient Protection and Affordable Care Act (PPACA) into law (Luther Hart, 2014). As written, the PPACA will be the most extensive change in the financing and provision of healthcare in 50 years (Luther Hart, 2014). The stated purposes of the legislation are to decrease the number of medically uninsured people as well as decrease the cost of insurance and healthcare for those already insured (Shi Singh, 2015). Medicaid expansion is significant element of the PPACA and is designed to provide health insurance to the lower income population (Vincent Reed, 2014). Medicaid program expansion and the individual mandate are two of the primary elements of healthcare insurance and finance reform (Vincent Reed, 2014). Additionally preventative health services delivered without cost, removal of health insurance exclusions for preexisting conditions, allowing children under the age of 26 to remain on their parent’s healthcare insurance, and gradually decreasing the out of pocket costs for Medicare Part D participants are mandated by the provisions of the PPACA (Vincent Reed, 2014). Medicaid Expansion Expansion of the Medicaid program is an important part of the PPACA (Vincent Reed, 2014). To decrease the number of Americans without health insurance coverage, the Medicaid expansion is designed to provide coverage for individuals with incomes of up to 138% of the federal poverty level (FPL). Individuals with incomes of 100% to 400% of the FPL, who did not qualify for Medicaid, would be eligible for subsidies in the Insurance Marketplaces mandated by the passage of the PPACA (Sommers, 2015) . In the states that do not expand Medicaid, Sommers (2015) notes that individuals who do not qualify for Medicaid and have incomes below the criteria to receive insurance subsidies in the healthcare marketplace will remain uninsured. Cognitive Psychology is the study of the human mind; it is how we reason, decide, and produce and comprehend language. In order achieve this we use both mental representations and mental processes. Like in any other situation, much of these cognitive functions are being used in the Sherlock’s Holmes YouTube video â€Å"Dinner with Mary Morstan and Watson†. At the beginning of the clip, you can hear the noise of the restaurant. Holmes closes his eyes, then shortly afterwards Watson calls his name. With these sources I plan on honing my writing skills so that I not only have the business knowledge but I will also be able to present this knowledge in an organized and professional format. This comes in very handy for checking current status and staying on schedule. While these links are very valuable my peers are also a very valuable tool. The Value of Learning TeamsThere is much value in forming teams. The opportunity exists to create improved processes, competitiveness, increased quality, better communicatio ns, higher morale, more creativity, better problem-solving, and better decisions. By using teams their will be more sources of different ideas, experiences, and skills. These differences allow the opportunity of finding more solutions with more creativity in the solutions. Teams also allow us to share our skills and knowledge with others allowing us to develop optimal solutions to problems as well as spotting more problems more quickly. This team effort will provide us with more ideas and alternate solutions, thus creating a better quality solution. There is always an inherent competitiveness in a team environment. This competitiveness will drive team member to higher productivity and more researching, which will lead to a more detailed decision. The quality of work will also increase because team members will take accountability for their work so nobody else can take credit for their work. This will lead to higher quality because after taking accountability for their work they will want to make sure they provide complete details in a timely manner so they do not look bad in front of their peers. This is also of high value because we will see how others approach the same problems to lead us to alternate solutions, which leads us into the values of problem-based learning throughout our journey. The Value of Problem-Based LearningThe value of problem-based learning is that we can use real world problems to learn from. By using these real-world problems we can apply our knowledge directly to a realistic scenario. By using realistic examples and solving real problems, we will become more adept to problem-solving outside of the classroom. This provides the opportunity to use the tools we are learning about in a hands-on manner. This approach will help us to identify problems more quickly, look at the whole picture and develop multiple solutions, and make a decision that will benefit all parties involved. There are several tools we can use to help us make the optimal decision. They are: brainstorming, alternative solutions, analyzing alternatives and assessing the risks that surround it, and selecting the best solution. Medicines: I used to take medicines only when I am not well at my young age, even I refused to do so n recover automatically but now I need to take medicines on regular interval to keep myself ready to live routine life. Mind: Now my mind is mature and practical enough to understand every little thing, so as during my young age I never back step to accept challenges but now I am little bit afraid just because of my age factor. Another key to stay healthy and keep the heart healthy is by being happy always and keeping the stress away from me. After 50 most of the people is suffering from presbiopia while after 80 by the cataract. Heart: Heart becomes less efficient comparatively which results in loss of stamina and can result into serious problems in future. However, to prevent this at first it is necessary to be aware with our health. Iam trying to bring some much needed changes in my life and settling into a new life style. I sleep properly, exercise in the proper amount and I eat only healthy food. I eat healthy. I take orange for fibre, vitamin C, low fat cheddar cheese and avocado slices for lunch with a low fat yogurt parfait with blueberries, strawberries and bananas for dessert. I forgot about the nuts for snacking, nuts are nutritious and supposed to contain the good kind of fat. I forgot about the two cups of veggie crunch sticks each afternoon maybe I need to cut those veggie crunch sticks out. Ill eat more high fibre and other foods that are supposed to help lower LDL. I am excited about the changes Ive made. Its good time to revisit my goals which I have already set during my adulthood. As I was very busy in work and earning money, running in race of settling myself I left few things which I love to do. So I have already planned to do all these things after my retirement. Richard B. Sewall claims that Melville's vision in Moby-Dick is a cruel reminder of the original terror, in which all moral judgements are accompanied by tensions, paradoxes, and ambiguities. In response to this statement, I agree that all moral judgements are fraught with tensions, paradoxes and ambiguities. Much of Ishmael's experiences while on land and at sea deal with making moral judgements; the act of forming an opinion by discerning what is right and wrong. Melville uses Ishmael to prove his vision that moral judgements are derived from (life) experiences directly affected by tensions, paradoxes, and ambiguities. Melville uses excellent representations of how tension can impact moral judgement making. Ishmael undergoes a particular situation in which tension directly affects his process of analyzing and judgement making. There is an obvious tension between Captain Ahab and the crew of the Pequod due to Ahab's silent intensity and self-concentrated desire to kill Moby-Dick. Ahab seems to be in his own world, loosing himself to the temptations of getting revenge on the White Whale. Ishmael makes note of this unspoken tension while looking for him during his watch. ...I instantly gazed aft to mark if any strange face were visible; for my first vague disquietude touching the unknown captain, now in the seclusion of the sea, became almost a perturbation...but whatever it was of apprehensiveness or uneasiness-to call it so-which I felt...(Melville, 109). Although Ishmael had not seen Ahab yet, he found it peculiar that he remained secluded in his quarters below the deck. This instance creates tensi on in Ishmael's mind, making him second-guess his attendance aboard the Pequod. This tension was the perfect recipe to help Ishmael decide how much he would want to interact with Ahab. In other instances, Melville uses the whale to show how paradoxes can affect judgement. When Stubb kills a whale in Chapter 61, Ishmael lingers around the incident, explaining first exactly what the dart is and what the crotch is. When Ishmael wants to examine representations of whales, he looks at monstrous and less erroneous pictures of whales and then depictions of whales in paint, teeth, wood, sheet-iron, stone, mountains, and stars. This commitment to analyzing the whale in general creates a paradox in that Ishmael either underestimates or overestimates the whale and its characteristics. It is a constant contradiction that affects Ishmael's judgement in a way that makes him feel either at ease or unrest with the whale. Melville states the ambiguity of experience in the chapter Queequeg in his Coffin. When Queequeg is seized by fever, he orders his coffin in the shape of a canoe. When he recovers, he uses it as a storage chest and an object of art. Later, the coffin is used as a life preserve/buoy for Ishmael. The general idea Melville portrays is that the meaning of an object is determined by an individual, and not in itself. Melville uses Ishmael as the direct link to Queequeg and his coffin when the ship sinks. The different perceptions of the coffin more or less deal with foreshadowing, and Ishmael's desire to analyze the future and judge for himself what is best for him. Melville had many ways of portraying how moral judgements can be impacted. I believe Sewall's claim that all moral judgements are fraught with tensions, paradoxes, and ambiguities to directly apply to moral judgement, as Melville portrayed this through Ishmael and particular events that took place in the novel. Background and Impact of The Civil Rights Act of 1964 The fight against racial injustice did not end after the passage of the  Civil Rights Act of 1964, but the law did allow activists to meet their major goals. The legislation came to be after President Lyndon B. Johnson asked Congress to pass a comprehensive civil rights bill. President John F. Kennedy had proposed such a bill in June of 1963, mere months before his death, and Johnson used Kennedys memory to convince Americans that the time had come to address the problem of segregation. Background of the Civil Rights Act After the end of Reconstruction, white Southerners regained political power and set about reordering race relations. Sharecropping became the compromise that ruled the Southern economy, and a number of African-Americans moved to Southern cities, leaving farm life behind. As the black population in Southern cities grew, whites began passing restrictive segregation laws, demarcating urban spaces along racial lines. This new racial order eventually nicknamed the Jim Crow era did not go unchallenged. One notable court case that resulted from the new laws ended up before the Supreme Court in 1896, Plessy v. Ferguson. Homer Plessy was a 30-year-old shoemaker in June of 1892 when he decided to take on Louisianas Separate Car Act, delineating separate train cars for white and black passengers. Plessys act was a deliberate decision to challenge the legality of the new law. Plessy was racially mixedseven-eighths whiteand his very presence on the whites-only car threw into question the one-drop rule, the strict black-or-white definition of race of the late 19th-century U.S. When Plessys case went before the Supreme Court, the justices decided that Louisianas Separate Car Act was constitutional by a vote of 7 to 1. As long as separate facilities for blacks and whites were equal separate but equal Jim Crow laws did not violate the Constitution. Up until 1954, the U.S. civil rights movement challenged Jim Crow laws in the courts based on facilities not being equal, but that strategy changed with Brown v. Board of Education of Topeka (1954) when Thurgood Marshall argued that separate facilities were inherently unequal. And then came the Montgomery Bus Boycott in 1955, the sit-ins of 1960 and the Freedom Rides of 1961. As more and more African-American activists risked their lives to expose the harshness of Southern racial law and order in the wake of the Brown decision, the federal government, including the president, could no longer ignore segregation. The Civil Rights Act Five days after Kennedys assassination, Johnson announced his intention to push through a civil rights bill: We have talked long enough in this country about equal rights. We have talked for 100 years or more. It is time now to write the next chapter, and to write it in the books of law. Using his personal power in the Congress to get the needed votes, Johnson secured its passage and signed it into law in July 1964. The first paragraph of the act states as its purpose To enforce the constitutional right to vote, to confer jurisdiction upon the district courts of the United States to provide injunctive relief against discrimination in public accommodations, to authorize the Attorney General to institute suits to protect constitutional rights in public facilities and public education, to extend the Commission on Civil Rights, to prevent discrimination in federally assisted programs, to establish a Commission on Equal Employment Opportunity, and for other purposes. The bill prohibited racial discrimination in public and outlawed discrimination in places of employment. To this end, the act created the Equal Employment Opportunity Commission to investigate complaints of discrimination. The act ended the piecemeal strategy of integration by ending Jim Crow once and for all. The Impact of the Law The Civil Rights Act of 1964 did not end the civil rights movement, of course. White Southerners still used legal and extralegal means to deprive black Southerners of their constitutional rights.
